Benefits of fetal surgery to repair spina bifida persist through school age, NIH study finds

BETHESDA, Maryland, Jan. 24, 2020 /PRNewswire-HISPANIC PR WIRE/ — Children as young as 6 years old who underwent fetal surgery to repair a common birth defect of the spine are more likely to walk independently and have fewer follow-up surgeries, compared to those who had traditional corrective surgery after birth, according to researchers funded by the National Institutes of Health. Their study appears in Pediatrics.

The procedure corrects myelomeningocele, the most serious form of spina bifida, a condition in which the spinal column fails to close around the spinal cord. With myelomeningocele, the spinal cord protrudes through an opening in the spine and may block the flow of spinal fluid and pull the brain into the base of the skull, a condition known as hindbrain herniation.

In 2011, the Management of Myelomeningocele study, funded by NIH’s Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD), found that by 12 months of age, children who had fetal surgery required fewer surgical procedures to divert, or shunt, fluid away from the brain. By 30 months, the fetal surgery group was more likely to walk without crutches or other devices.

For the current study, NICHD-funded researchers re-evaluated children from the original trial when they were 6 to 10 years old. Of the 161 children who took part in the follow-up study, 79 had been assigned to prenatal surgery and 82 had been assigned to traditional surgery. Children in the prenatal surgery group walked independently more often than those in the traditional surgery group (93% vs. 80%). Those in the prenatal surgery group also had fewer shunt placements for hydrocephalus, or fluid buildup in the brain (49% vs. 85%), and fewer shunt replacements (47% vs. 70%). The group also scored higher on a measure of motor skills.

The two groups did not differ significantly in a test measuring communication ability, daily living skills, and social interaction skills.

“Prenatal surgery for myelomeningocele carries benefits and risks, compared to traditional postnatal surgery,” said Menachem Miodovnik, M.D., of the NICHD Pregnancy and Perinatology Branch. “This study provides important information for physicians with patients who are considering prenatal surgery.”

The study was conducted by Amy Houtrow, M.D., Ph.D., of the University of Pittsburgh, and colleagues.

The All of Us Journey is an interactive mobile exhibit designed to share information about the All of Us Research Program, which aims to collect health data from one million or more people living in the United States––with the goal of accelerating precision medicine.

An individual’s personal health can be affected by factors such as geographical location, occupation, ethnicity, genetic health history and more. By gathering health data from one million or more participants, from all walks of life, All of Us hopes to build a data resource rich with diversity—particularly from groups who have historically been underrepresented in biomedical research—in order to give researchers a more complete picture of factors that impact health and disease.

Since the tour’s launch in the summer of 2017, the Journey has traveled to more than 38 states and 110 communities across the country, helping enroll more than 7,000 participants in the program. Currently, 8,820 people in Texas are among the more than 319,000 people who have already enrolled in the All of Us Research Program.

Low-dose aspirin may reduce preterm birth risk among first-time mothers

BETHESDA, Maryland, Jan. 24, 2020 /PRNewswire-HISPANIC PR WIRE/ — Daily low-dose aspirin, from as early as the sixth week of pregnancy through the 36th week, may lower the risk for preterm birth among first-time mothers, suggests a study funded by the National Institutes of Health. The clinical trial, which involved more than 11,000 women in several low- and middle-income countries, found that women taking daily low-dose aspirin were 11% less likely to deliver before the 37th week of pregnancy, compared to those given a placebo.

The study was conducted by Matthew K. Hoffman, M.D., of ChristianaCare in Newark, Delaware, and colleagues in the Global Network for Women’s and Children’s Health Research, a clinical trials network funded by NIH’s Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD). It appears in The Lancet.

“Our results suggest that low-dose aspirin therapy in early pregnancy could provide an inexpensive way to lower the preterm birth rate in first-time mothers,” said study author Marion Koso-Thomas, M.D., of NICHD’s Pregnancy and Perinatology Branch.

Preterm birth is the most common cause of infant death and the leading cause of long-term neurological disability in children. According to the study authors, advances in newborn care have improved survival for preterm infants, but this care is limited or unavailable in many parts of the world. Earlier studies have suggested that low-dose aspirin may reduce the risk of preterm birth and preeclampsia, a potentially life-threatening blood pressure disorder of pregnancy. However, these studies were not large enough to statistically determine the therapy’s effectiveness in reducing preterm birth.

The researchers enrolled 11,976 women with a first-time pregnancy from seven sites in India, Pakistan, Zambia, Democratic Republic of the Congo, Guatemala and Kenya. Roughly half were assigned at random to receive 81 milligrams of aspirin daily; the other group received a daily placebo. Women were included in the study only if they maintained a pregnancy for more than 20 weeks.

Preterm birth (before 37 weeks) occurred in 11.6% of the women who took aspirin and in 13.1% of the women who took the placebo. Similarly, birth before 34 weeks (early preterm delivery) occurred in 3.3% of the aspirin group and 4% of the placebo group (a 25% reduction). Women in the aspirin group also had a lower rate of perinatal mortality (stillbirth or newborn death in the first seven days of life), compared to the placebo group (45.7 per 1,000 births vs 53.6 per 1,000 births). The risk of high blood pressure disorders of pregnancy at term did not differ significantly between the groups.

The authors note that the low cost and safety of low-dose aspirin therapy suggest that it could be easily adapted for widescale use.
