On Lok PACE Honored with Age-Friendly Health System — Committed to Care Excellence Recognition

SAN FRANCISCO, March 10, 2025 /PRNewswire-HISPANIC PR WIRE/ — Announced today, as part of the Age-Friendly Health Systems movement, On Lok PACE has achieved special recognition as an Age-Friendly Health System — Committed to Care Excellence.

Part of the Institute for Healthcare Improvement’s (IHI) ongoing efforts to ensure age-friendly care for all older adults, the recognition has been given to organizations that performed strongly over a three-month analysis of a set of evidence-based elements of high-quality care, known as the 4Ms: What Matters, Medication, Mentation, and Mobility.

As of August 2023, more than 1,900 hospitals, practices, convenient care clinics, and nursing homes have been recognized as Age Friendly Health Systems – Committed to Care Excellence. On Lok PACE is proud to join the growing ranks of these organizations in putting the 4Ms into practice. Globally, more than 2,480,000 older adults have been reached with 4Ms care.
